Breast Implant Placement: Over vs. Under the Muscle

When you’re considering breast augmentation, choosing your implant size and shape may feel like the biggest decision you’ll make. But there’s another important part of planning your procedure: where the implant will be placed.

Breast implants can generally be positioned either above the chest muscle or at least partially beneath it. Each approach can create beautiful, natural-looking results, but the right choice depends on your anatomy, the amount of natural breast tissue you have, your desired look, and other factors your plastic surgeon will evaluate.

Understanding the differences can help you feel more prepared for your breast augmentation consultation and the decisions you’ll make along the way.

What Does Breast Implant Placement Mean?

Your breast implant is positioned in a carefully created pocket within the breast. When people talk about going “over” or “under” the muscle, they’re referring to the implant’s relationship to the pectoralis major, the large muscle that covers the upper chest.

With over-the-muscle placement, also called subglandular placement, the implant sits beneath the breast tissue but above the chest muscle.

With under-the-muscle placement, the implant is positioned at least partially beneath the pectoral muscle. Depending on the surgical technique, the muscle may cover primarily the upper portion of the implant rather than the entire implant.

Neither placement is automatically better. Your surgeon considers how each option will work with your existing anatomy and the result you hope to achieve.

What Are the Benefits of Over-the-Muscle Breast Implants?

Placing an implant above the muscle can be a good option for some patients, particularly those who already have enough natural breast tissue to provide adequate coverage over the implant.

Because the chest muscle is not lifted to accommodate the implant, this approach may involve less disruption of the muscle. It can also avoid the implant movement that can sometimes occur when the pectoral muscles contract.

However, the amount and quality of your existing breast tissue matter. In someone with relatively little natural tissue, an implant placed above the muscle may be easier to see or feel, particularly around its edges. This is one reason your surgeon will carefully assess your breast tissue, skin, and proportions before recommending placement.

What Are the Benefits of Under-the-Muscle Breast Implants?

Placing an implant partially beneath the chest muscle provides an additional layer of tissue over the upper portion of the implant. For patients with less natural breast tissue, this added coverage can help soften the transition between the chest and breast.

Under-the-muscle placement is often considered when the goal is a smooth, natural-looking upper breast contour. It may be especially helpful for patients with thinner tissue who want to minimize the visibility of implant edges.

Since the procedure involves the chest muscle, patients may experience more tightness or discomfort in the early stages of recovery. The implant can also temporarily appear higher on the chest before the tissues relax and the breasts settle into their final position. Your surgeon will explain what to expect based on the specific technique used for your procedure.

Does Implant Placement Affect How Natural Your Results Look?

It can, but placement is only one part of the equation.

Natural-looking breast augmentation depends on the relationship between the implant and your individual proportions. Implant size, profile, shape, existing breast tissue, skin quality, chest dimensions, and surgical technique all influence the final result.

For example, someone with minimal natural breast tissue may benefit from additional implant coverage, while another patient may already have enough tissue to create a smooth contour with an above-the-muscle approach.

At New Horizons, breast augmentation planning is highly individualized. Our goal is to enhance your shape in a way that complements your frame and reflects the look you have in mind.

Is Recovery Different for Over- vs. Under-the-Muscle Placement?

Your recovery experience will depend on the details of your surgery, although implant placement can play a role.

Because under-the-muscle placement involves the pectoral muscle, patients may notice more pressure, tightness, or soreness through the chest during the initial recovery period. Above-the-muscle placement does not require the same degree of muscle involvement.

Every patient heals differently, so your recovery timeline should be based on your surgeon’s instructions rather than someone else’s experience. We’ll talk with you about activity restrictions, follow-up care, and what you can expect as swelling improves and your implants settle.

How Do I Know Which Breast Implant Placement Is Right for Me?

You don’t need to have this figured out before your consultation. In fact, implant placement is best decided after your surgeon has evaluated your anatomy and understands what you want your results to look like.

During your consultation, we’ll consider factors such as:

  • The amount of natural breast tissue you have

  • Your skin and tissue characteristics

  • Your chest and breast dimensions

  • Your desired breast size, shape, and projection

  • Your implant choice

  • Your lifestyle and aesthetic preferences

  • Any existing asymmetry or changes related to pregnancy, aging, or weight loss

We can also use computer imaging to help you explore breast augmentation options and visualize how different implant choices may complement your proportions. While imaging cannot guarantee an exact surgical result, it can be a helpful tool for communicating your preferences and making decisions with greater confidence.

A Personalized Approach to Breast Augmentation

There are plenty of decisions involved in breast augmentation, but you don’t have to make them on your own. Implant placement should be selected thoughtfully, with your anatomy and long-term aesthetic goals in mind.

At New Horizons Center for Cosmetic Surgery, we take the time to understand what you want from breast augmentation, evaluate the details that can influence your result, and develop a surgical plan specifically for you. Whether you’re looking for a subtle enhancement or more noticeable fullness, our focus is on creating balanced, natural-looking results that feel right for your body.
